Joseph Detailed Update Monday Evening -Tue, Jan 17, 2017 at 12:39 AM

This group will get more details as I am able to record them.  My hope is that it will keep you up to date.  Probably don't forward, but if someone wants to know these details, let me know and I will add them to the list.

Summary:
Joseph has improved today.  The air in his chest has largely cleared.  The O2 being provided by the ventilator has been dropped from 100% down to ~40% (GOOD) . However, he remains sedated to prevent him from fighting the ventilator. On several drugs to manage blood pressure and keep blood going to the lungs.  No estimate on how long he will be in.  

Timeline: 
10am: Rounds - Team Meeting with Docs and others. 
           Terms 
           Learnings
1:00 pm:  start to lower O2 -by 3pm is down to 44%
8:15 pm:
    • Measured Head 35 cm length 55?, weight 8+ lbs mostly water
    • Did other “cares”, lifted him off the bed for weights
    • Responded with increased breathing
    • Gave Atavist as sedative to calm down from cares
    • prior was 0 breaths / minute, post: 77 breaths/minute, remained high until 9:30 when returned to 0/min 
9:30 
    • Dopamine dropped 1 microgram to 12 
Answers:
     The only thing we know is that he air is gone.
Questions:
  1. Does he still have tiny holes in his lungs…probably
  2. How long will he need to remain on the ventilator
  3. He is limp and non-responsive due to sedation.  Can’t tell if other issues exist. Like Does he have other problems with the nervous system
  4. When will he be able do start eating? 
Drugs
  • Dopamine 12 mg
  • Lipids and other TPN
  • DT-10
  • Nitric Oxide : 20 mm
  • Broad Spectrum antibiotics for first 48 hours (until wednesday ~noon)
  • Morphine
  • Meliphine - Blood pressure and improve heart function
Terms:
  • Thymus: Organ that provide TCells and helps with immune system.  Is rather large in young infants. 
  • PRF: As Needed
  • TPN: Total Patient Nutrition
  • Cares: Things done to ensure all is well. e.g. clean mouth, check each IV, etc, etc.
